Eligibility Requirements

Be 60 years old or older

Be an Illinois resident and a U.S. citizen or permanent resident

Have applied for Medical or be eligible

Individuals with physical disabilities also qualify

Income level does not affect eligibility for the program; however, it is evaluated to determine whether the client may contribute to the cost of services.

Activities and Services Include

Physical exercise and yoga classes

Breakfast, lunch, and snacks

Medical monitoring and health education

Games, arts, and painting

Free round-trip transportation

Income level does not affect eligibility; it is only used to determine possible cost-sharing.
